Required documentation includes:
- proof of residency in the Hillsboro School District
- birth certificate,
- social security number,
- immunization record,
- child custody orders (if applicable),
- transcript and/or report card from previous school (include current IEP and most recent evaluation, if applicable), and
- discipline report from previous school(s).
**Proof of Residency: may be a utility bill that has your name and address on it; if you are living with extended family or just moved into an apartment, we will need a notarized renter/leaser form turned in-(this form will be provided for you when you come in for the appointment). If you are moving into a newly constructed home, a copy of your contract will suffice.**
